Submitting Your Presentation-Symposium Faculty & Annual Meeting Invited Speakers

2012 Symposium Faculty
In preparation for the symposium, the below items you should submit. These items will help ASNR meet CME requirements, and make sure your audio-visual needs are met.
- Disclosure Statement Form
- Audio Visual Request Form
- PowerPoint Presentation (Final version preferred but not required)
- Bibliographic Information for Non-members
- Questions for eCME (email to ASNR)
- ECME Requirement and release form
- W-9 Form ( non-member – US only)
Symposium Faculty: You will not be required to prepare a formal chapter or syllabus. Rather, we would appreciate your PowerPoint (in electronic file format) presentation in advance of the meeting by Friday, April 6. In the past, we have printed handouts for meeting attendees in advance of the meeting. In 2012, ASNR is streamlining the process and the handout will become an electronic link.
Please submit by Friday, April 6 the version of your PowerPoint you wish for attendees to view during the meeting.
2012 Invited Speakers
In preparation for the meeting, the below items you should have submitted. These items will help ASNR meet CME requirements, and make sure your audio-visual needs are met.
Member:
-Disclosure Statement (In order to speak, all presenters must disclose in advance per ACCME requirements) Speaker at CME events must disclose even if no disclosure exists.
-eCME Release Form
-Presentation Summary (150-400 words)
-Questions (two (2) multiple choice) – (Need to email the questions, do not add to Oasis system)
Required by ACCME to for use within electronic Continuing Medical Education (eCME) activity.
Non-member:
-Disclosure Statement (In order to speak, all presenters must disclose in advance per ACCME requirements) Speaker at CME events must disclose even if no disclosure exists.
-eCME Release Form
-Presentation Summary (150-400 words)
-Bibliography (limit of 8 within the system)
May be added to allow attendees to further study the topic presented
-Questions (two (2) multiple choice)
Required by ACCME to for use within electronic Continuing Medical Education (eCME) activity.
-W-9 Form (required US)
-Registration & Housing and Travel Expenses
