Annual Meeting Invited Speakers

Information must be completed by all invited speakers
-Disclosure Statement Form or submit to www.cmedisclosure.com or https://secure4.inet7.com/gotomylist/cmeDisclosure/EM2/index.cfm?id=86
-Audio Visual Request Form
-eCME Permission to Use Material Form

Complete by
ASNR Members:
-Registration Form
(Advance registration and housing deadline was Friday, April 18, 2008)

Non-members:
- W-9 Form (for domestic U.S. speakers only)
- Travel Form
- W-8 Form(for Canadian and international speakers)

Speakers will be able to submit their PowerPoint presentation online before the ASNR meeting. You may submit your presentation(s) anytime to Freeman Audio Video Solutions' (ASNR’s contracted audio visual company) website at www.presentationmanagement.com/ASNR.

An email address and Presentation/Unique Number is required for access. You may submit your presentation beginning Saturday, March 1, 2008. The deadline for advance submission is 5:00pm (CST) on Friday, May 30, 2008. You may return to the website at anytime to adjust the PowerPoint or to re-submit a new file.
