The American Society of Neuroradiology is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians. The American Society of Neuroradiology takes responsibility for the content, quality, and scientific integrity of this CME/CPD activity.

The American Society of Neuroradiology designates NER Foundation Symposium educational activity for a maximum of 10.00 Category 1 credits towards the AMA Physician's Recognition Award. Each physician should claim only those credits that he/she actually spent in the educational activity.

The American Society of Neuroradiology designates ASNR Annual Meeting educational activity for a maximum of 33.25 Category 1 credits towards the AMA Physician's Recognition Award. Each physician should claim only those credits that he or she actually spent in the activity.

CME/CPD CERTIFICATE

Continuing Medical Education (CME)/Continuing Professional Development (CPD) certificates for the number of hours earned during the NER Foundation Symposium 2004: Integration of Imaging Strategies in Neuroradiology & ASNR Annual Meetiong will be mailed to each registrant within 4-6 weeks following the meeting. Please contact the ASNR Headquarters Office at (630) 574-0220 or fax (630) 574-1740 with any address changes to ensure receipt of your CME/CPD certificate.

LETTER OF ATTENDANCE

If you require a Letter of Attendance, please request at the Registration Desk located in the South Lobby (Level 4) of the Washington State Convention & Trade Center during the Annual Meeting.
