2004 Registration Policies

 Policies and Deadlines

Policy	Before Friday, April 30	After Friday, April 30
Advance Registration (Deadline is 5:00p PST, Friday, April 30) Replacement Badges on-site, $15 each		$75 Late Fee
Registration Changes Changes must be made in writing.	No administration fee.	Changes that result in a refund will be charged a $50 administration fee.
Cancellations	$50 cancellation fee	Between Saturday, May 1 and Friday, May 21, cancellations will results in a $100 cancellation fee. After Friday, May 21, there will be no refunds for cancellations.

 Mailing of Badges

• Name Badges will be mailed to all North American attendees who are registered prior to Saturday, May 1, 2004.
• Name Badges will be mailed to all international attendees who are registered prior to Friday, April 2, 2004. International attendees who register after Friday, April 2, 2004 will be sent confirmation of registration via fax or email.
• Attendees who do not receive their name badge prior to the meeting may pick-up it up, along with their meeting materials, at the Express Registration desk.
• Bring your name badge and registration confirmation receipt to the meeting. Requests for a new badge are subject to a $15 replacement fee.

 Registration Services On-Site

• The ASNR Registration area will be located in the South Lobby - (Level 4) of the Washington State Convention & Trade Center.
• Express Registration - Registered attendees should bring the registration confirmation receipt received in their Advance Registration Packet to the Express Registration desk during posted hours to pickup registration bag and the Proceedings Book.
• ASNR Registration Desk - On-Site registration services will be available during these hours.

Sunday, June 6	6:00am-6:00pm

Thursday, June 10	6:00am-6:00pm
Friday, June 11	6:00am-11:45am

 Registration Changes

• All changes received after Friday, April 30, 2004 are subject to a $50 administrative fee.

 Meeting Cancellations

• Cancellations made prior to Friday, April 30, 2004 are subject to a $50 administrative fee.
• Cancellations made between Saturday, May 1 and Friday, May 21, 2004 are subject to a $100 cancellation fee.
• No refunds for cancellations made after Friday, May 21, 2004.

 Refunds

• Refunds will be issued 6-8 weeks after the close of the meeting.
• Refunds will not be issued for no-shows.

 All registration changes and cancellations must be made in writing.
